Raw milk (doodh) is the predominant form of milk consumed in Pakistan. The milkman (dodhi) system — in which small producers collect and deliver fresh, unprocessed milk directly to consumers at the doorstep or from roadside collection points — is deeply embedded in both urban and rural areas across the country. This informal infrastructure supplies the majority of Pakistan's milk to consumers without pasteurization.

The Pakistan Standards and Quality Control Authority (PSQCA) and provincial food authorities including the Punjab Food Authority (PFA) and Sindh Food Authority set hygiene and quality standards for commercial dairy operations, which technically require pasteurization for packaged milk sold through formal retail channels. In practice, enforcement applies primarily to large organized dairies and packaged milk brands rather than to the widespread informal farm-direct and milkman distribution system. No separate consumer raw milk authorization framework exists, as raw distribution has historically been the norm rather than a regulated exception.
