The sale of raw milk for human consumption in South Africa is governed by the Milk and Dairy Products Regulations (R1555 of 21 November 1997), made under the Foodstuffs, Cosmetics and Disinfectants Act, 1972 (Act No. 54 of 1972). Under Regulation 3(1), raw milk may only be sold in the areas of jurisdiction of local authorities listed in Annex C of R1555. A local authority that considers it can exercise sufficient inspection control may apply to the Minister of Health, through its provincial health department, to be added to Annex C. Coverage is patchy and determined municipality by municipality rather than at a national or provincial level.

Raw milk offered for consumption must meet the conditions in Regulation 4: no antibiotic or antimicrobial residues above prescribed limits, no pathogenic organisms, a standard plate count not exceeding 50,000 CFU per 1ml, a negative clot-on-boiling test, E. coli and coliform bacteria within prescribed limits, and somatic cell count within prescribed limits. The milk must be packed in a closed container and carry a label clearly bearing "Unpasteurised" / "Ongepasteuriseerd" or "Raw milk" / "Rou melk". The producing herd must be enrolled in the Bovine Tuberculosis Scheme and the Bovine Brucellosis Scheme established under the Animal Diseases Act, 1984 (Act No. 35 of 1984).

Raw milk also circulates informally in rural and peri-urban areas beyond the formally authorized municipalities, and enforcement of the regulations outside licensed facilities is widely described as poor. South Africa has a small but active raw milk consumer market, concentrated among health-conscious urban consumers and farming communities.
